[Your Business Name] — Photo, Video, and Media Release

Please read this agreement carefully before you sign it. When you sign, you give up certain legal rights, including the right to sue.

1. Permission

I give [Your Business Name] (the "Business") permission to photograph and record me, and to use my name, image, likeness, and voice in photos, video, and audio (the "Media").

2. How the Media may be used

The Business may use, edit, and publish the Media in any format, including on its website, social media, advertising, and printed materials, worldwide and without a time limit. I will not receive payment for this use. I do not need to approve the final Media.

3. Ownership

The Business owns the Media. I release the Business from claims related to the use of the Media as this release allows, including claims for invasion of privacy or right of publicity, to the extent the law allows.

4. Withdrawal

I may withdraw this permission for future uses by writing to the Business at [your email]. Withdrawal does not affect Media that is already printed or published.

5. Minors

If I sign for a minor, I confirm that I am their parent or legal guardian and I give this permission for them.

6. General terms

This agreement is intended to be as broad and inclusive as the law of the place where the Activity takes place allows. If any part of it is found invalid, the rest stays in full force. This agreement binds me, my heirs, and my legal representatives. It is the entire agreement between me and the Business about these matters.

I have read this release. I understand it, and I sign it voluntarily.
