Tattoo Consent & Release Form

Please complete this form before your appointment. Bring a valid photo ID.

[Your Business Name] — Tattoo Consent, Release, and Waiver

Please read this agreement carefully before you sign it. When you sign, you give up certain legal rights, including the right to sue.

1. My consent

I ask [Your Business Name] (the "Studio") and its artist to apply a tattoo to my body. I confirm that I am at least 18 years old and that I will show a valid photo ID. I confirm that I am not under the influence of alcohol or drugs.

2. A tattoo is permanent

I understand that a tattoo is a permanent change to my skin. Removal, if possible, can be expensive and painful, and it may leave scars. I understand that the colors and design may look different on my skin, and that they may change over time.

3. Design, spelling, and placement

I have checked the design, the spelling of any words, and the placement on my body. I approve them. The Studio is not responsible for a design, spelling, or placement that I approved.

4. Health and risks

I have told the Studio about all medical conditions that may affect the tattoo or my healing, including diabetes, heart conditions, epilepsy, hemophilia or other bleeding disorders, skin conditions, allergies (including to latex, ink, or ointments), and medications such as blood thinners. I confirm that I am not pregnant or nursing.

I understand that tattooing has risks, including pain, bleeding, bruising, swelling, infection, allergic reactions to ink or materials, scarring, and uneven healing. I understand that the Studio uses single-use needles and sterile procedures, but that no procedure is completely without risk.

5. Aftercare

I have received aftercare instructions. I understand that my healing depends on how well I follow them, and that the Studio is not responsible for problems caused by poor aftercare. I will see a doctor if I see signs of infection, such as increasing redness, heat, swelling, pus, or fever.

6. Release of liability

To the fullest extent the law allows, I release, waive, and discharge the Business and its owners, employees, instructors, contractors, volunteers, and agents (the "Released Parties") from all claims, demands, and liability for injury, illness, death, or loss of or damage to property that arise from my participation in the Activity. This includes claims caused by the ordinary negligence of the Released Parties. This release does not apply to gross negligence or intentional misconduct.

7. Indemnity

I agree to indemnify and hold harmless the Released Parties from any claims, losses, costs, or expenses, including reasonable attorney fees, that result from my actions, my breach of this agreement, or my violation of the rules.

8. Photos

I agree that the Studio may photograph my tattoo and use the photos in its portfolio and marketing, unless I tell the artist that I do not agree.

9. General terms

This agreement is intended to be as broad and inclusive as the law of the place where the Activity takes place allows. If any part of it is found invalid, the rest stays in full force. This agreement binds me, my heirs, and my legal representatives. It is the entire agreement between me and the Business about these matters.

I have read this agreement. I understand it, and I sign it voluntarily. I understand that I give up substantial rights by signing it, including my right to sue.
