[Your Business Name] — Body Piercing Consent, Release, and Waiver

Please read this agreement carefully before you sign it. When you sign, you give up certain legal rights, including the right to sue.

1. My consent

I ask [Your Business Name] (the "Studio") and its piercer to perform the piercing described below. I confirm that I am not under the influence of alcohol or drugs, and that I have eaten in the last 4 hours.

2. Health and risks

I have told the Studio about all medical conditions, medications, and allergies that may affect the piercing or my healing, including metal allergies, diabetes, bleeding disorders, and blood thinners.

I understand that piercing has risks, including pain, bleeding, swelling, infection, allergic reaction, keloids or scarring, migration or rejection of the jewelry, and nerve damage. I understand that healing times vary, and that some piercings take many months to heal.

3. Jewelry

I understand that the first jewelry is chosen for safe healing. I will not change it until the piercer says that the piercing has healed enough. The Studio is not responsible for problems caused by jewelry that I change or buy elsewhere.

4. Aftercare

I have received aftercare instructions and I will follow them. I will contact a doctor if I see signs of infection.

5. Release of liability

To the fullest extent the law allows, I release, waive, and discharge the Business and its owners, employees, instructors, contractors, volunteers, and agents (the "Released Parties") from all claims, demands, and liability for injury, illness, death, or loss of or damage to property that arise from my participation in the Activity. This includes claims caused by the ordinary negligence of the Released Parties. This release does not apply to gross negligence or intentional misconduct.

6. Indemnity

I agree to indemnify and hold harmless the Released Parties from any claims, losses, costs, or expenses, including reasonable attorney fees, that result from my actions, my breach of this agreement, or my violation of the rules.

7. Signing for minors

If I sign for one or more minors, I confirm that I am their parent or legal guardian. I agree to the terms of this agreement for them and for myself. I accept the risks of the Activity for them, and I agree to the release and indemnity above on their behalf to the extent the law allows. I agree to supervise them as the Business requires.

8. General terms

This agreement is intended to be as broad and inclusive as the law of the place where the Activity takes place allows. If any part of it is found invalid, the rest stays in full force. This agreement binds me, my heirs, and my legal representatives. It is the entire agreement between me and the Business about these matters.

I have read this agreement. I understand it, and I sign it voluntarily. I understand that I give up substantial rights by signing it, including my right to sue.

Questions about this template

Can a parent sign for a minor's piercing?

Many places allow some piercings for minors with a parent present, and others do not. Turn on minors only if your local law allows it, and add any ID rules to the text.

Is this form different from the tattoo consent form?

Yes. It covers jewelry, healing times, and metal allergies, which are specific to piercing.
